- For EXPRESS and TAILORED customization levels, you can include your request or existing customization solution from our catalog in the notes section of your standard wheelchair order form. Prefer using a dedicated form? Our EVO Custom Request Form is also available for all customization levels and helps ensure nothing gets missed.
- For SIGNATURE requests, or if you are not sure where your idea lands, the EVO Custom Request Form is strongly recommended to help us assess your needs and provide accurate timeline and pricing.
